ABC for Health, Inc. and HealthWatch Wisconsin
Check It Out! Promoting Access to Wisconsin’s HealthCheck Program for Children
Check it Out! will promote increased use and understanding of HealthCheck services, a vital, but dramatically underused Medicaid feature for low income children in Wisconsin. A multi-pronged awareness campaign will use social media, webinars, and printed materials to promote understanding and proper use of HealthCheck in Wisconsin.

Boys & Girls Club of the Tri-County Area
Parent University
Parent University will offer parent education workshops in the areas of literacy, health awareness, ACES, mental health, addiction, disease prevention, and healthy relationships. Each month, the program will offer a new training topic in collaboration with experts designated in the fields, including: UW Extension, Catalpa Health, ThedaCare, & Green Lake County Health and Human Services. Transportation, child care, and a meal will be provided to remove all barriers for parents wanting to attend.

Children’s Hospital of Wisconsin Foundation
Reach Out and Read (ROR) Wisconsin
Reach Out and Read (ROR) Wisconsin is the early literacy initiative of Children’s Health Alliance (Alliance) of Wisconsin, a program of Children’s Hospital of Wisconsin. ROR gives young children a foundation for success by incorporating books into pediatric care and encouraging families to read aloud together. During well-child visits, children receive a new book to take home and parents receive a prescription for sharing books with their child.

Strong Baby Sanctuary
The Strong Baby Sanctuary Program reduces infant mortality by promoting healthy births and healthy families. Churches and other community partners serve as Sanctuaries for women in need, connecting them to services such as prenatal and parenting information; instruction on safe-sleep practices and on reducing the risk of Sudden Infant Death Syndrome; and emotional support. The services include the Blanket of Love program of group education and resources.

ACES garden classes
Amount Requested: $5,458
The goal for this project is to boost resilience in elementary school aged children with adverse childhood experiences (ACEs) through skills training in the school gardens. This program will provide opportunities for students to engage in hands-on nutrition education with a positive role model specifically aimed to build resilience.

Healthnet Of Rock County, Inc.
Enhanced Diabetes Management Program for the Latino Population
The Enhanced Diabetes Management Program for the Latino Population (EDMPL) helps to improve the health of Rock County’s low income Latino population through an approach that employs cultural sensitivity and breaks down language, literacy, and cultural barriers. This project will offer an innovative and unique approach to diabetes prevention and diabetes management for the Latino community; modeled around the diabetes triad by focusing on exercise, nutrition, and medication/disease management.

Madison Area Care for the Homeless OneHealth (MACH1)
Madison Area Care for the Homeless OneHealth Street Medicine Team
The Street Medicine Team is one of several active programs within the MACH1 organization. Since March 2018, the Street Medicine Team has done weekly rounds on the State Street corridor to provide a controlled scope of direct clinical services to people experiencing homelessness, improve their trust of health care providers [and by extension the local health care system(s)] and enhance patient health literacy. Other MACH1 programs include a bi-monthly Foot Care Clinic and Health Needs Assessment.

Let’s Talk About Pain Medicines
MCHC will hold ten workshops on the ‘Let’s Talk About Medications’ that the Wisconsin Health Literacy developed. MCHC will hold these workshops at various locations in the Greater Milwaukee area over a one-year time period. MCHC will deliver the workshops in multiple languages including: Arabic, Urdu, Turkish, Albanian, Hindi, Urdu, Burmese and Rohynga. The workshops will be delivered to approximately 200 immigrant and refugee community members from the Greater Milwaukee Area.

Providers and Teens Communicating for Health (PATCH)
The Providers and Teens Communicating for Health (PATCH) program aims to improve the communication and relationships between teens and health care providers. PATCH brings teens from diverse backgrounds together in a youth-driven education and empowerment model with outreach to peers and providers. A PATCH Central WI team was created in July 2018. Because of its promising impact on teens and providers, will deliver PATCH workshops in rural areas of central and northern WI.

Safe Community Coalition of Madison and Dane County
Zero Suicide and Peer Recovery Support Services
Safe Communities is requesting a grant to expand our highly successful peer recovery coach services to support people screened by health systems as at risk of suicide. This pilot program would identify people with lived experience – those who attempted suicide and are in recovery, and/or who lost loved ones to suicide; train them as Peer Support Specialists, and match them with patients referred by area health care systems adopting Zero Suicide practices.

St. Clare Memorial Hospital HSHS
Nurse’s Nook Program
The Nurse’s Nook Program addresses the health risks faced by children living in rural communities. School nurses support the education of students while addressing their physical, mental, emotional, and social health needs. Funds are available to assist with transportation to medical appointments and the purchase of medical equipment. In addition, children and families will have access to basic needs items that help improve health such as food, clothing, and hygiene products.

UW Department of Obstetrics and Gynecology
Statewide Summit on Innovations in Contraceptive Care
The UW Dept. of Ob-Gyn will collaborate with the WI Contraceptive Access Network to host a Statewide Summit on Innovations in Contraceptive Care. The purpose will be to bring together a diverse group of stakeholders and providers to learn about the latest evidence-based strategies to increase pregnancy intendedness and reduce the proportion of rapid repeat pregnancies. We will provide hands-on skill-based sessions in placement of long-acting reversible contraceptive methods to 40 clinicians.

Providers and Teens Communicating for Health [PATCH] – Coulee Region
Will establish PATCH-Coulee Region: a youth-driven health prevention program that engages, educates, and empowers adolescents and adults to become trusted partners in care, improve adolescent well-being, and ultimately reach optimal health, safety, and economic security.decisions for the incorporation of more physical activity in their daily lives and for the selection of healthy food items.
