The Wisconsin Medical Society Foundation is pleased to announce grant awards for the following 2020 projects:
Bayview Foundation, Inc.
Healthy Kids Program
Bayview Foundation, a low-income housing community located near downtown Madison, will grow and enhance its youth health and wellness programming by implementing youth gardening, healthy cooking classes, and food and health literacy program for school-age children. Partnerships with Troy Farm, REAP Food Group, Edgewood College Nursing, and Community Groundworks will ensure a strong and meaningful program that is engaging and culturally relevant.

Cultivating Youth - Garden Program
Boys & Girls Club of Janesville will manage a neighborhood garden plot to grow thier health and wellness programming, through the addition of hands-on activities and fresh produce to use produce for the summer snack program, and to host community events each spring and fall.

Boys & Girls Club of the Wausau Area
Trauma-Informed Training for Four Central Wisconsin Boys & Girls Clubs
About 50 staff from Wausau, Antigo, Stevens Point, and Wisconsin Rapids Boys & Girls Clubs will participate in a trauma-informed training to reduce or mitigate adverse childhood experiences (ACEs). Funds will support a one-day collaborative training in a centralized location. Club staff will learn to identify, assess, and implement practical strategies to help Club youth build resiliency and feel safe. Another training element will include coping strategies to reduce compassion fatigue.

Bridge Lake Point Waunona Neighborhood Center
Finding Balance Fall Prevention Program
Finding Balance is a holistic fall prevention program that aims to reduce falls among older adults. Community members will (a) improve their strength and balance, (b) learn about what increases the risk of falls, (c) create individualized action plans to avoid them, and (d) meet with healthcare professionals, including a physical therapist and pharmacist who will teach and monitor safe exercises and review medications.

Care Net Pregnancy Center of Dane County
Health Education, Prevention & Treatment of STIs
We will provide a fun and innovative enhancement to already existing comprehensive sex education curriculums within schools that focuses on sexual risk reduction behaviors and STI prevention and education. Their accredited clinic will then provide free and confidential testing and treatment of prevalent STIs; a mobile clinic which serve underserved neighborhoods. Expedited Partner Therapy will be provided with the goal of reducing new and recurring infections.

Healthier You
The Healthier You program seeks to influence patient behavior in a positive way for all those with, or at high risk for developing, either condition. They use an intervention model that combines clinic care with nutrition counseling, physical activity partnerships, incentive fitness and nutrition packages and regular one-on-one check-ins, and support from our Healthier You coordinator, a CMA and CHM.

Blanket of Love Sanctuary
Blanket of Love Sanctuary reduces infant and maternal mortality by promoting healthy women, healthy births, and healthy babies in Milwaukee. It serves vulnerable expectant mothers and women who have given birth – wrapping them in a blanket of group prenatal and parenting education and support, and connecting them to resources including primary care, food, housing, and transportation. Grant will support Blanket of Love Sanctuary at three hospitals, serving 50 high-risk women.

Dunn County's Zero Suicide Initiative in Schools
Dunn County Human Services is leading the Dunn County's Zero Suicide initiative in Dunn County. Grant will provide suicide prevention training of QRR [Question, Persuade, and Refer] to 85 Dunn County Human Services staff and 572 Dunn County school employees.

La Crosse County Prevention Network Coalition – Coulee Council on Addictions Fiscal Agent
Youth and Community Education – decreasing the stigma of addiction and mental health
La Crosse County Prevention Network is requesting funds to provide for programming and activities to help reduce the stigma around addiction and mental health. Grant will bring the Nationally known group Rise Together to La Crosse County and provide programming for schools as well as a community presentation. The program will also provide for a 21-day health challenge for the youth and a youth ambassador program to be started in each school.

Madison Area Care for the Homeless (MACH) OneHealth
Madison Area Care for the Homeless (MACH) OneHealth Community Health Needs Assessment (CHNA)
The MACH OneHealth CHNA is a vital source of information for understanding the healthcare needs and barriers of people experiencing homelessness in Madison. The CHNA data will improve current, and develop new, community-driven programming. Their Foot Care, Street Medicine, and Care Connections clinics are a direct result of their first CHNA done in 2016. While local organizations have conducted CHNAs in recent years, only MACH OneHealth has directly engaged homeless folks for a CHNA.

Medical College of Wisconsin
Stepping On
Stepping On is an evidence-based 7-week program designed to help adults in Wisconsin reduce falls and stay active. Per WIHA, the program has been demonstrated to reduce falls by 50% and to reduce ED visits due to falls by 70%. Additional Stepping On instructors will be trained in central Wisconsin in order to implement additional programs in Langlade, Lincoln, Marathon, and Wood counties, which will allow for increased year-round enrollment and decreased fall rates.

Muslim Community and Health Center
Diabetes Prevention in Immigrant and Refugee Populations
MCHC will hold 10 workshops on "Diabetes Prevention." MCHC will hold these workshops at various locations in the Greater Milwaukee area over a one-year time period. MCHC will deliver the workshops in multiple languages including: Arabic, Urdu, Turkish, Albanian, Hindi, Urdu, Burmese and Rohynga. The workshops will be delivered to approximately 200 immigrant and refugee community.

Rural Wisconsin Health Cooperative
Wisconsin's Northern & Central (WiNC) Graduate Medical Education (GME) Consortium Project
The WiNC GME Consortium plans to increase access and quality of health care to the northernmost thirty-seven counties in Wisconsin by creating a regionally focused GME administration infrastructure designed for GME growth. The main goal of the Consortium is to become an Accreditation Council of Graduate Medical Education (ACGME) sponsoring institution in order to stabilize existing GME programs in the targeted area and create new GME positions throughout the region.

Let's Talk About Pain Medicines
Wisconsin Health Literacy (WHL) will organize10 educational workshops, to adults in select communities throughout Wisconsin. Participants will learn how to safely and effectively use prescription opioids while learning other pain treatments as an alternative to taking opioids. WHL will seek Wisconsin Medical Society (WMS) member physicians to facilitate the workshops. This project continues the work from the successful pilot "Let's Talk About Pain Medicines" project sponsored by WMS in 2018-19.

Youth Action Hudson

In the fall, Youth Action Hudson youth volunteers (Hudson High School students) will develop and facilitate educational presentations to all 5th grade classrooms in the district, both public and private.These presentations will focus on emotional health topics such as: self-confidence & self-image, Positive behaviors and choices.In the Spring, YAH youth develop informative presentations on relevant social topics to all 6th, 7th, and 8th grade students located at the Hudson middle School.

Increasing Health Literacy and Access to Pre-Exposure Prophylaxis (PrEP) for Minority Communities within Milwaukee through the Stay PrEP’d Up Campaign.
The AIDS Resource Center of Wisconsin (ARCW) will launch the second phase of the Stay PrEP’d Up campaign in Spring 2020. This educational campaign will encourage African American and LatinX men who have sex with men (MSM) between the ages of 25 - 34 living in Milwaukee to take control of their sexual health. The initiative will incorporate current Milwaukee PrEP users to help educate and raise awareness of PrEP among their peers and those most vulnerable to HIV in the LGBTQ+ community.
